Summary: The scope of this consultancy is to provide advice on the expansion of the Brisbane North PHN Program, to build capacity in the Program staff and to design and conduct the evaluation. The key question for the evaluation is: Is the provision of SHSH to patients with complex chronic conditions presenting to public hospitals in the Brisbane North region over the period of the program, delivering similar or improved health outcomes at lower cost than the provision of usual care?
