Summary: Capital Health Network is seeking an Expression of Interest from an evaluation institution who is interested in the planning and evaluation of the pilot, home multi‐disciplinary ACT Breathlessness Intervention Service. This project will aim to increase the level of symptom management support for patients in the ACT who have a life limiting illness with advanced disease and are suffering intractable breathlessness by funding a pilot program of a home multi‐disciplinary breathlessness intervention service. This pilot program is anticipated to run for 12‐18 months, which includes the development of the model of care and data evaluation. The ACT Breathlessness Intervention Service project is based on the Cambridge Breathlessness Intervention Service and the South Australian Community Breathlessness Intervention Services which address the Breathing ‐ Thinking ‐ Functioning domains by utilising a multi‐disciplinary approach including physiotherapy, medical, psychosocial and occupational therapy input.
