Summary: Safe@home, will be co-designed, patient and family centred using a prospective, non-blinded pragmatic randomised controlled trial to evaluate home telemonitoring supported by primary care for patients living with chronic disease in low socioeconomic neighbourhoods, evaluating the effectiveness and cost effectiveness of reducing emergency department (ED) presentations (ambulance ramping), readmission (hospital avoidance), out-of-hospital death and improving access to primary care (clinic block) and self-care.
